Breakthrough Session Contract Agreement

This Breakthrough Session Contract Agreement (the “Agreement”) is entered into between you (the “Client”) and Unlimited Health Coaching, referred to as the “Unlimited Health Team.” This Agreement outlines the terms and conditions under which the Client will participate in a breakthrough session provided by the Unlimited Health Team.

1. Purpose of the Breakthrough Session: The Unlimited Health Team offers breakthrough sessions to provide health and life coaching services to the Client. These sessions are designed to help the Client overcome challenges, set achievable goals, and enhance their overall well-being.
2. Scope of Services: During the breakthrough session, the Unlimited Health Team will provide coaching, guidance, and support to the Client. This may include discussing health goals, identifying obstacles, and developing an action plan to progress towards desired outcomes.
3. Confidentiality: The Unlimited Health Team recognizes and respects the confidential nature of the information shared by the Client during the breakthrough session. The Unlimited Health Team agrees to maintain the privacy and confidentiality of all information disclosed by the Client, except as required by law.
4. Disclaimer of Liability: If the Client has been directed to the Unlimited Health Team’s platform from a course via another business, the Unlimited Health Team and Unlimited Health Coaching disclaim any liability and hold no association with that business. The Client acknowledges that the Unlimited Health Team is an independent entity and the breakthrough session is separate from any other courses or businesses that may have referred the Client.
5. Client’s Responsibility: The Client understands that the breakthrough session is not a substitute for professional medical or mental health advice, diagnosis, or treatment. The Client takes full responsibility for their own health and well-being decisions and acknowledges that any actions they take based on the breakthrough session are at their own risk.
6. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which the independent contractor collaborating with the Unlimited Health Team is based. In the event of any disputes arising out of or in connection with this Agreement, the parties agree to submit to the exclusive jurisdiction of the courts in that jurisdiction.
7. Term and Termination: This Agreement will become effective on the date of the breakthrough session and will remain in effect until the session is completed. Either party may terminate the Agreement at any time by providing written notice to the other party.
8. Entire Agreement: This Agreement constitutes the entire understanding between the Client and the Unlimited Health Team regarding the breakthrough session and supersedes all prior agreements, understandings, or representations, whether written or oral.

 

By e-signing below, the Client acknowledges that they have read and understood the terms and conditions of this Breakthrough Session Contract Agreement and agree to be bound by them.
